CMS selected ACO participating in the Shared Savings Program.
The ASPA Connected Community (ASPA-CC | Administered by the Arizona State Physicians Association) in Phoenix is available to all ASPA members and allows you to exchange clinical data with any other participating provider or healthcare facility.
The Arizona Care Network (ACN) from Dignity Health is available to all ASPA members via the ASPA-CC. The ACN allows you to participate with other community providers in potentially lucrative payer contracts in return for improved care, outcomes and better managed beneficiary costs afforded by the clinical integration programs. You must commit to the terms of the ACN program. This is a great example of what you can do with this ASPA-CC capability.
